Things To Note

• Please complete this application if you wish to surrender a dog to Wags and Walks. 

• In addition to this application, please email photos of the dog and all medical records/history to surrender@wagsandwalks.org - applications will only be considered if photos and medical records are received.

• If you are surrendering your dog for medical or surgical reasons, we recommend first applying for the Medical Relief Program through Wags for Good.

• Please note: we are very careful to only rescue dogs that are friendly/comfortable/social with people of all ages and other dogs. Please only complete this application if you believe your dog will pass our temperament evaluation.
